Vertigo Therapy: How to use Vestibular Rehabilitation & Maneuvers to eliminate Positional Vertigo & BPPV

Over the past 6 years CardioFlex Therapy has successfully treated hundreds of vertigo patients, and assisted them in eliminating their dizziness. I want to share some concepts of how to use Vestibular Rehabilitation to get maximum results for Benign Paroxysmal Positional Vertigo (BPPV) patients.

Diagnosis
The first step in getting rid of dizziness begins with a solid diagnosis. Does the patient have a peripheral or a central problem? What causes the patient to get dizzy? Is there any spinning vertigo with the dizzy episodes? Patients that have Meniere’s Disease or other chronic conditions will get recurring bouts of dizziness, so helping them resolve each episode quickly will be the best case scenario for those individuals.


Central Vertigo involves brain and spinal cord lesions such as tumors, cysts, surgeries, scar tissue, and degeneration. Neuromuscular disorders such as Multiple Sclerosis, Ataxia, Stroke, & Parkinson’s Disease can affect the cerebellum in the brain, causing gait and balance disturbances as well as severe vertigo. Peripheral Vertigo is typically considered to be caused by anything not in the central nervous system (brain and spinal cord). Much of the time, Peripheral Vertigo is synonymous with Positional Vertigo (BPPV).

To truly diagnose the vertigo as peripheral, the patient must have a brain MRI to rule out central involvement. A Hallpike Dix test and a Supine Roll Test are performed to see if the patient has Positional Vertigo on the left side, the right side, both sides, or not at all. Positional Vertigo (BPPV) is caused by displaced otoconia, commonly called crystals, which tend to get stuck in the 3 semicircular canals. The Hallpike Dix Test will be positive if crystals are stuck in either the Anterior or Posterior canals. The Supine Roll Test will be positive if crystals are stuck in the Horizontal canals.

Eye tests need to be conducted with and without head movement to see if there is a visual sensory-in component. In other words, if a patient gets dizzy from visual tests with or without head movement, then there is probably a visual sensory-in pathway which can trigger the symptoms at any time, usually seen with severe vertigo. If the patient gets any nystagmus (spinning vertigo), then they have a sensory-out component through their eyes, and are more likely to have a sensory-in component as well.

Treatment

At CardioFlex, we believe in a concept that we call “Repetitive Maneuvers.” We believe that several maneuvers should be performed in each treatment session in order to clear the crystals out of the semicircular canals, and become free flowing like they were before getting stuck. There are several maneuvers that we like to use in our sessions to clear the crystals, such as the Brandt-Daroff Maneuver, the Semont Maneuver, Epley’s Maneuver, and the Barbeque Roll Maneuver.

The Brandt-Daroff Maneuver is the best maneuver to give to patients for a Home Exercise Program (HEP), because it’s the simplest maneuver to execute. In our clinic, we will have the patient do this maneuver first to get the crystals flowing, and get their vestibular system ready to handle more intense stimuli. After this maneuver, we will do eye exercises in sitting with the patient, if they have any sensory-in problems. If they don’t have any sensory-in dizziness, then we will proceed to static and dynamic balance exercises, with and without head movements. The head movements are either very small rapid movements, or slow full range movements, which are meant to disturb balance and aggravate the dizziness symptoms. After these balance exercises are done, we may decide to do some functional activities with head movements, such as squats, lunges, and different directional walking exercises. The goal of each therapy session is to aggravate the dizziness and clear the displaced crystals.

Which exercises we choose for the patient is based on a progressive loading pattern, starting off very easy then progressing to very difficult exercises by the end of therapy. At the end of each treatment session, we will do 1 power maneuver, such as the Semont Maneuver, Epley’s Maneuver, or the Barbeque Roll. Once they can handle a power maneuver without any adverse dizziness symptoms, then we will change the order of the maneuvers to enable as many as 4 maneuvers per session. The ideal order becomes: Brandt-Daroff Maneuver, Semont Maneuver, balance exercises, functional exercises, and lastly the Epley’s Maneuver.

The Brandt-Daroff, Semont, and Epley’s Maneuvers clear crystals in the Anterior and the Posterior semicircular canals, but not in the Horizontal (Lateral) canal. The Barbeque Roll Maneuver clears crystals in the Horizontal canal, so it’s really not needed unless the other maneuvers are not able to completely get rid of the vertigo, or if the patient tested positive for the Supine Roll Test (Horizontal BPPV test). When the Barbeque Roll test is incorporated, it should either be the last maneuver, or the 2nd to last maneuver performed, to get maximum results. If the Barbeque Roll test is either too violent for the patient to handle (gets sick, nystagmus, etc.) or the patient cannot physically do it (rolling over), then another Horizontal Maneuver can be used such as the Log Roll Maneuver or the Gufoni Maneuver.

ACL Tear Rehab Exercises versus ACL Tear Reconstruction

ACL RehabTearing your ACL (Anterior Cruciate Ligament) can be a traumatic episode or a slow degeneration over time. Either way, there are only two reasonable choices: get an ACL repair surgery or just do Physical Therapy rehab exercises to try to stabilize the injured knee. It’s difficult to have a completely stabilized knee with a full ACL tear, because of what the ACL does within the knee joint.
The ACL is one of four major ligaments in the knee joint. The other three are the MCL (Medial Collateral Ligament), the LCL (Lateral Collateral Ligament), and the PCL (Posterior Collateral Ligament). The ACL is a strong ligament that prevents anterior translation of the tibia on the femur. In other words, when you squat or bend your knee in a closed-chain position (foot on the ground), the ACL prevents or limits the lower leg from moving forward.
Knee ligaments are usually torn by severe forces caused by extreme and rapid bending of the knee. Since ligaments are strong bands of soft tissue that attach one bone to another, tearing a major ligament is usually very painful and elicits a “popping” sensation. Occasionally, the ACL can just degrade gradually over time, perhaps occurring with several small accidents or injuries. People with a deficient ACL feel tremendous instability with squatting, lunging, jumping, and quick change of direction exercises. ACL tears are more common in high contact sports such as football and rugby, in which awkward hits and movements occur. Sports with rapid change of direction such as basketball and soccer also have a higher than normal occurrence rate for ACL tears. Auto accidents, slip and falls, and jumping awkwardly can all cause the ACL to tear.

Diagnosis
The best way to diagnose an ACL tear is using either the Anterior Drawer Test, or the Lachman’s Test. These tests help the provider to evaluate the amount of anterior translation of the tibia on the femur. A positive test occurs when there is hyper-mobility of the tibia greater than a normal amount of joint play in the knee. If there is no definitive end-feel during these tests, then a full ACL tear is highly likely. Of course, an MRI will need to be performed to fully confirm the diagnosis.
Once the diagnosis has been confirmed, the patient has to decide whether to get a surgical reconstruction, or just do Physical Therapy rehab to learn how to deal with it. If surgery is opted for, its usually performed in one of two different ways. The first and most common way of replacing the torn ligament is to use the ACL from a cadaver body, and implant this ACL where the old one was attached inside the knee. The second way to do this repair is to graft a pseudo ACL off one of the patient’s hamstring tendons, and attach it inside the knee.

ACL RehabRehab
If the surgery goes well, then the patient will usually begin Physical Therapy within the first week, so that the joint doesn’t become stiff. Sometimes, the surgeon will order Home Physical Therapy for the first 2-3 weeks after the surgery, because it’s so much easier for the patient. Each Orthopedic Surgeon typically has their own ACL post-op protocol, and some surgeons actually have a few or several different protocols, depending on the complexity of the surgery and the patient’s age and physical conditioning level. For example, a 20 year old professional athlete will be on a faster protocol than a sedentary 50 year old, that never plays sports or exercises. Although a protocol may be important, it’s also up the to Physical Therapy provider to decide if the patient is ahead or behind where they are supposed to be, and adjust the exercise routine accordingly. Providers have to remember that each individual is different, and the road to health is paved with ups and downs. With time, good medical care, and personalized Physical Therapy, patients can expect to minimize their healing time significantly.
The basic post-op exercise protocol following ACL reconstruction typically is drafted for a 6-9 month time frame. The Physical Therapy usually lasts anywhere from 3 months up to 9 months, depending on patient motivation, complications, and need to regain previous level of function. Sometimes patients can continue to do the exercises on their own, once they get to a good level in the clinic. Week 1 therapeutic exercises may include Quad Sets, Straight Leg Raises, Hip exercises, Hamstrings Stretches, and Heel Slides. Eventually, the patient will be able to perform Squats, Lunges, Deadlifts, Knee Extensions, Plyometrics, and eventually Running and Agility Drills.
The rehab exercises for patients choosing not to get surgery are similar to the ACL reconstruction post-op exercises, except instead of starting at week 1, the ACL deficient patient would incorporate some early exercises and also perform many closed chain exercises to work on knee stability. Squats, Lunges, Deadlifts, and Balancing exercises can be utilized immediately, as long as the patient doesn’t have too much pain or inflammation left over from the initial trauma of the ACL tear.

Long Term Prognosis
Although it seems unlikely that ACL deficient patients can get back to playing sports at a high level, it is possible to develop surrounding muscles to compensate for the torn ACL. Also, wearing an ACL brace during sports activities helps with stability for both the ACL deficient and post-op patient. The choice of whether to get the ACL reconstruction surgery or not can only be made by the patient. Ultimately, Physical Therapy rehab exercises are recommended either way, but it is possible to regain a fairly good amount of function without going through the surgery and the difficult rehab. But typically, if the patient wants to resume playing sports or activities at their prior level before the injury, then the surgery is strongly recommended.

New Additions to the Outpatient Clinic

CardioFlex Therapy is proud to announce some new additions to the outpatient clinic which will expand our repertoire of therapeutic treatments and customer care.

Our patients with neuropathy have already reported positive results after only a few sessions with our new Anodyne Infrared Light Therapy system! The Anodyne device uses infrared light to stimulate local circulation and decrease pain and stiffness, safely and without the use of drugs. We’re excited to have this treatment available to our patients!



The Knee/Ankle Flexionator is an overpressure stretching mechanism designed to increase range of motion and flexibility. The Flexionator is extremely effective speeding the recovery from knee injuries such as ACL reconstructions and meniscus repairs.

How to Stay in Shape During the Holidays

Most Americans would like to stay in shape during the holidays, but usually don’t understand how to do it. This article outlines a plan to help accomplish this goal.

Start by taking a look at your current lifestyle and eating habits, and make general decisions around what you’re doing. There are 2 main factors to consider in your approach, nutrition and exercising, but there are many questions that you need to ask yourself in order to formulate a basic strategy.

Are you happy with your current weight, or are you trying to lose weight? Do you exercise regularly, or are you sedentary? How long and how intense are your workouts?

You need to build a plan without too much instant shock to your system, and you have to take into account the increased seasonal calories that you will be taking in. I would recommend trying to eat a moderate diet, and increasing your exercise program to combat the extra holiday food that you may eat. If you don’t think you have the time to increase your workouts then you can choose to either decrease your caloric intake, or increase the intensity of your exercise routine. If time allows, I would definitely recommend increasing your cardio workouts during this time of the year.

If you do cardio for 1 hour, 5 to 6 times a week, and add light weights with high reps 3 times a week, then you should be able to get through the Holidays without any weight gain, unless you’re eating chocolate and french fries every day! But if you haven’t been working out at all, then scale down to doing cardio just 3 to 4 times a week, and weights twice a week.

If you’re an athlete or former athlete and can do sports training or more intense activity such as plyometrics, then mixing these into your routine instead of cardio will actually burn more calories. With sprints and plyometrics, a 5:1 rest to work ratio is ideal for training. That may sound like too much rest, but you will need the break time to be able to go all out. The human body is not designed to sustain peak intensity for very long.

I recommend weighing yourself everyday in the morning to hold yourself accountable, and to also know where you stand in relation to your goals. If you are weighing yourself everyday, then you will also know what works and what doesn’t work, and how certain foods and exercise programs affect your weight. Remember, no matter how much you workout, if you eat more calories than you burn, then you will probably gain weight. Not all calories are created equal, so try to limit your saturated fats, trans fats, carbs, and sugars. Try your best not to eat any of these after 7pm, for maximal fat burn throughout sleep.

Just remember that after the holidays are over, your New Year’s resolutions don’t have to be about weight loss and getting into shape, because you’ll already be well on your way there!

Office Worker Stretching Guide

A majority of Americans spend 40 hours a week, 8 hours a day tied to a desk and staring at a computer. The downfall of computer based work is that it compromises the well being of your body. Not only does prolonged sitting have an effect on cardiovascular health but also musculoskeletal integrity as well. Prolonged sitting along with bad posture can shorten hamstrings and hip flexors, contribute to neck and back pain, and even diminish core strength. Maintaining Balance

Balance is not commonly viewed as an important aspect of physical fitness but it can prove essential in preventing injury. We normally don't think about our balance until we slip or fall and get injured because of instability. Just like anything else such as muscle and bone strength our balance can deteriorate the older we get. Balance is one of those "use-it-or-lose-it" situations, it must be trained and strengthened over time. Poor balance can lead to instability and put us at risk to slip or fall causing serious injury such as a broken hip, which in turn, can lead to prolonged injury and a downward spiral into dependence and ill health. The good news is that it's never too late to start, simple exercises that don't require a lot of equipment can be utilized to help improve balance.
